Federal Services Membership

When applying for Federal Services Membership:

1. You MUST be a member in good standing with the American Optometric Association (AOA) through your state association or AFOS to apply.

2. Please complete all areas of the application.

3. In addition, please remit with your application the appropriate Federal Services member fee based on the schedule below:

January 1	$100 annual fee
April 1	$75 prorated fee
July 1	$50 prorated fee
October 1	$25 prorated fee
