Registration for the 2011 Colorado Vision Summit is available online or by mail. Please note that no registrations will be accepted by fax, email or telephone.

Each doctor or paraoptometric registrant must fill out a separate registration form.

Pre-registration rate deadline is Tuesday, June 8, 2011. If registering by mail, your payment envelope must be postmarked by June 8, 2011 to qualify for the discounted pre-registration rate.

• If you register online, you must print a confirmation form; otherwise, your registration is NOT guaranteed.
• Online registrations submitted on or before 9:59pm MDT, June 8, 2011, qualify for the early-bird discounted rate. Registrations submitted after June 8 will be processed at the on-site rate.
