Registration for the 2008 Colorado Vision Summit is available online or by mail. Please note that no registrations will be accepted by fax, email or telephone.
Each doctor registrant must fill out a separate registration form. Spouse registrants may register on the same form as the attending doctor.
When registering online, multiple paraoptometrics may register on one form. When registering by mail, each paraoptometric registrant must fill out a separate registration form.
Pre-registration rate deadline is Wednesday, June 11, 2008. If registering by mail, our payment envelope must be postmarked by June 11, 2008 to qualify for the discounted pre-registration rate.
To Register Online:
- If you register online, you must print a confirmation form; otherwise, your registration is NOT guaranteed.
- Online registrations submitted on or before 11:59pm MDT, June 11, 2008, qualify for the early-bird discounted rate. Registrations submitted after June 11 will be processed at the on-site rate.
To Register By Mail:
- Click here to download a conference brochure which includes a mail-in registration form on page 19.
- Complete the registration form.
- Payment in full must accompany the registration form. Registrations received without payment will not be processed.
- Registrations postmarked after June 11 will be processed on-site in Steamboat Springs and will be processed at the on-site rate.
- Your postmark will determine the rate that you are charged.
