Registration is available online or by mail. Please note that no registration forms will be accepted by fax, telephone or email. Each registrant must fill out a separate registration form. Click here to go to the registration page.

The early registration deadline is Wednesday June 8, 2011.

To qualify for the early-bird rate, you must successfully submit an online registration by 9:59PM on Wednesday June 8, or your mail-in form must be received by the CVS Registrar postmarked no later than June 8. All registrations received after June 8 will be processed on-site at the higher registration rate.

Cancellation Policy

Refund of CVS registration fee(s) less $25 per person for administrative costs will be granted on written request including reason for cancellation only if postmarked by June 23, 2011. No refunds will be made for cancellations after June 23, 2011. Registrations are not transferable.

Course Outlines

After you have registered, you will receive an email that includes a special link to a web page where you can download course handouts prior to attending the Summit.

In addition, all doctor and paraoptometrics will be provided with a CD of these course outlines when you check in at the meeting registration desk. Printed outlines will not be provided at CVS. It is highly recommended that you access and download or print the handouts before attending the Summit. A print station will be available on site.

Badge Admission

An official CVS name badge is required for entrance to all events. The loan or transfer of this badge is strictly prohibited; violation of the policy subjects the registrants to forfeiture of the right to attend, as well as the loss of all fees paid.

Exhibit Hall Entry

All CVS registrants and attendees (e.g. family members) may enter the exhibit hall at NO CHARGE. An adult must accompany children under the age of 12.

Lecture Rooms

In most cases, the lecture rooms will be set with tables and chairs. As a courtesy to the speaker and the attendees seated around you, we respectfully request that all cell phones and pagers be silenced during the lectures.

Literature Distribution

Registrants are not permitted to distribute literature, samples, souvenirs and the like in the lecture rooms and exhibit area.

Course Outlines on CD

Course outlines are provided on a CD to every doctor and paraoptometric registrant. Prior to the Summit and at the time you preregister, you will provided with a code to access a site in order to download the outlines. Replacement cost of the CD is $20.

Continuing Education Credit Hours for Doctors

The CVS OD program offers 34 hours of education/14 obtainable. The CVS does not guarantee that any courses are accepted for credit by a licensing board. It is the responsibility of attendees to be thoroughly familiar with the licensing requirements of their individual states.

A course verification form is provided when you enter the course within the first ten (10) minutes of the lecture. At the completion of the lecture, monitors are available to stamp the forms verifying attendance. A $25 service fee is charged per request for copies of CE credit slips.

One 50-minute lecture equals one (1) hour of CE credit.

Continuing Education Credit Hours for Paraoptometrics

Paraoptometric registrants seeking certification hours for ABO, NCLE, CPO, CPOA and CPOT must note that on the registration form and pay an additional fee of $5.
